Health Inspection

The total weighted all cycles score is based on the number, scope, and severity of deficiencies identified during the three most recent annual inspection surveys plus findings from the most recent 36 months of complaint investigations, all weighted by scope and severity. The total weighted all cycles score for the three surveys (plus complaints if any) is what determines the number of stars assigned for the health inspection 5 star rating, based on the cut points at the time of the survey. Adjusted Staff Hours

The staffing rating is based on two measures, RN hours per resident day and total staffing hours per resident day (RN + LPN + nurse aide).
